If you are a new patient to our office, the attached file contains our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. We try to simplify the preparation of your insurance claims, thereby holding down costs that are unrelated to the delivery of quality dental care to you and your family.
As a service to you, we will submit your insurance claims, provided you have supplied us with the necessary information. Your estimated portion is expected at each visit.
We work hard to provide this service to help you obtain as much as possible from your insurance benefit. However, insurance benefits vary, depending on your employer’s contract with your insurance company. Please remember that insurance is not a substitute for payment. Some insurance companies pay a fixed allowance for certain procedures, and others pay a percentage of the charge. It is your responsibility to pay any deductible amount, including co-insurance, or any other balance not paid by your insurance company.
Should you have financial problems which result in you delaying payment of your bill, please contact us. We will make every effort to work out an acceptable payment plan to enable you to take care of your obligation. All delinquent accounts will be pursued to the fullest extent allowed by law. Persistent nonpayment for services rendered will lead to termination of the doctor-patient relationship.
